Pet Walking Service Agreement

This agreement reviews with you the terms and conditions for pet walking services. Please review and sign at the bottom of the service agreement. You will receive a copy of the service agreement for your records.

Policies and Procedures

This form reviews the policies and procedures that will be followed as part of our service agreement for pet walking services. Please review and sign at the bottom of the policies and procedures form. You will receive a copy of the policies and procedures form for your records.

Veterinarian Release

Completing this form gives permission for a DogGone Days representative to transport your pet to your veterinarian or the closest facility (in cases of an emergency) in the event your pet requires medical attention. Please fully complete this form and we will be sure to fax a copy of it to your veterinarian so it can be place in your pet’s records. We will also email you a copy of this form for your records.

Price Sheet (informational)

The price sheet is for informational purposes and will help you understand the fees you will acquire from our pet services.

Client Profile

Completing this form will provide DogGone Days with information on how to contact you, gain access to your home, emergency contacts, special instructions, etc. This information will not be shared with anyone other than the DogGone Days representative servicing your pet. Please provide an email address so we can email the copies necessary for your records.

Pet Profile

Please complete a pet profile form for each pet you require pet services for. This form will provide DogGone Days with important information on each pet in your family. The form includes information such as habits, favorite places, leash location, feeding instructions, personality, dominance or aggressive issues we should know about, and any other special instructions you would like us to know while caring for your pet.

Bark Remarks

As part of our services, we will complete this form so you understand the services we provided on any given day we are caring for your pet including your pet(s) bathroom statistics, additional comments we have about the behavior of your pet(s), etc. In addition, you will be able to provide us any special instructions or comments in the space provided for the day.

Authorization and Contract Agreement

  1. Client authorizes DGD to perform pet care services as outlined in the Client Profile. Pet Profile, Veterinarian Release, and Policies and Procedures which shall become part of the contract.
  2. DGD agrees to provide the services stated in this agreement in a timely, reliable, caring and trustworthy manner. In consideration of these services, and as an express condition thereof, the Client expressly waives and relinquishes any and all claims against DGD, provided reasonable care and precautions are followed. Client hereby releases of any liability of any kind arising from pet(s). Client agrees to notify DGD with any complaints within 24 hours of end of services.
  3. Client authorizes DGD to obtain emergency veterinary care that may be necessary during time spent with Client’s pet(s). Client accepts responsibility for any charges related to this emergency care. DGD shall use the veterinarian indicated in the Client profile for this service. Should the Client’s veterinarian be unavailable, DGD is authorized to seek an alternative veterinarian. Every effort will be made to contact Client prior to obtaining emergency care.
  4. Client will provide proof that dog(s) is/are current on all legally required vaccinations.
  5. DGD will not be liable for any injury, disappearance, death, or fines of any pet(s) with unsupervised access to the outdoors.
  6. Client will be responsible for all medical expenses and damages resulting from an injury to the pet walker or other person(s) by the pet(s). Client agrees to hold harmless DGD in the event of a claim by any person injured by the pet(s). Client also agrees to disclose to DGD any pet(s) history involving injury to any human.
  7. DGD reserves the right to terminate this contract at any time, at its sole discretion: likewise, Client may terminate this contract at any time.
  8. It is expressly understood that DGD shall not be held responsible for any damages to Client’s property, or that of others, caused by the Client’s pet(s) during the period in which they are in its care. Client has advised DGD of all situations, which will relieve it of liability for damage.
  9. For vacation/travel care customers only: In the event that DGD is required to employ a locksmith to gain entry into Client’s premises due to a malfunction of the lock or key, it shall be the responsibility of the Client to reimburse for all costs incurred. The Client expressly gives DGD the authority to employ a locksmith on Client’s behalf in the event of the aforementioned occurrences. In addition, a fee amount based on the timeliness of the locksmith will be assessed due to the inconvenience incurred to DGD’s other clients.
  10. Client authorizes this contract to be valid approval for future services so as to permit DGD to accept telephone reservations and enter my premises without additional signed contracts or written authorizations.
  11. Client has read the terms of this agreement and confirms the accuracy of the information provided. Agreement is indicated by the signature below.

Interview and consultation

Prior to booking service from DogGone Days our representative will arrange a convenient time to meet with owner and pet(s). This allows everyone to become acquainted and for us to meet your pet(s) and answer any questions or address any concerns you may have. At that time we will go over the Policies and Procedures, contracts, instructions, complete forms, and acquire keys. For dogs, it is highly recommended that you allow the representative a brief opportunity to walk your dog outside of your residence at that time.

Reservations

Generally, we ask that you give a minimum of two weeks notice for services required. This way, your pet(s) will not be left alone. Availability is on first come, first served basis. The more notice we have, the better chance you have for booking your dates.

Dog walking

Daily dog walks are performed typically between the hours of 10:00 am and 4:00 pm. Every effort will be made to arrive at your residence at the time agreed to at booking. We allow a 90-minute window in case there is an emergency or delay. Payment for daily dog walking services is expected weekly on the last day of service for that week via a cash payment or by check payable to Rebecca Gaines. DogGone Days reserves the right to refuse service for non-payment. Services will continue as soon as payment is made.

Vacation/Travel care

Vacation/travel care for your pet(s) is commonly referred to as pet sitting. This service will be available upon request and subject to DogGone Days’ availability.

Collars, leashes, and tags

DogGone Days representatives will be more than happy to use your leash when they walk your dog. If you forget to leave one out for us, we will have one available. While in our care, a DogGone Days pet tag will be placed on your dog, which states that your pet is being cared for by us and includes our phone number. We feel this is a very important part of our service should your dog(s) get loose while you are unavailable to be reached.

Return check charge

Client agrees to pay a $25 fee for each check returned by the client’s bank regardless of the reason.

Holidays

There will be a $10 fee in addition to regular per visit or travel care fee charged for services on the following holiday/holiday weekends: New Years Day, Easter, Memorial Day, Fourth of July, Labor Day, Thanksgiving, Christmas Eve, Christmas Day, and New Year’s Eve. Holidays are subject to DogGone Days’ availability.

Keys/Access Code

At time of contract signing, you will need to provide DogGone Days with your preference for us to gain access to your home. If access is via a garage code or alarm system, an access code with specific instructions will need to be provided on the “Client Profile” form. If access is with a key, then you will need to supply DogGone Days with a working copy of the key.

Access to home

We understand that you may have relatives, neighbors, cleaning or repair services that could have access to your home while you are away or at work. DogGone Days is not responsible for any damages incurred to your home or pet by said persons or businesses. Please notify us if there will be anyone else having access to your home. We also reserve the right to decline services if you have made arrangements for someone else to care for your pet(s) while we are doing the same.

Unsecured pets

DogGone Days will not be held liable for free-roaming outdoor pets or pets left in backyards in the event of illness, injury, loss or death. It is highly advisable that all pets wear appropriate ID and that they remain inside your home or confined for their own safety and well-being in your absence.

All of our dog clients get treated to at least 25 minutes of undivided attention per visit, whether they are to be fed, taken for a walk, played with, or however else you wish us to spend time with your dog.
